1. Provide sexy mouse over changes to the cursor when using the top ruler.
Selecting an item on the top ruler turns cursor to a pointing
finger. Moving a mouse over a dragable item turns mouse pointer to a
"<=>".
Mouse over on the tab type toggle makes it an "exchange" icon. Other
platform maintainers should implement the new cursor types defined in
gr_Graphics.h to get these.

2. Right click on selected tabstop removes the tab stop.

3. Fix lots more screen dirt.

This fixes the screen dirt from bug 1119 plus lots from italic Time New
Roman "f".

Could people please hack away and let me know if they can reproducibally
produce any screen dirt?

I'll let you know if these can be easily fixed.
