I think this is appropriate because the image is being scaled.
"Image Size" sounds more like 'actual size' to me.

similarly we are setting image name, not using the filename or a taking a
name from the image metadata so again this seems appropriate.

"Alt" should not be abbreviated.
The users interface must leave space for localisation anyway so
shortening the word is not appropriate, and it also forces users to take
an extra fraction of a second to understand it.

Actually this should be changed to Description (Alt is based on an
implementation detail from HMTL).

The labels in this section are very wordy (you will have to forgive the
hypocrisy of me saying someone else is verbose). A radio list with four
items in it looks a little bit odd but I guess it is still short enough
that a drop down list is unnecessary.

I'd change it to

Text Wrapping
() None
() Right
() Left
() Justify

'Image Placement' doesn't seem perfectly right,
(or changed to Position,
the Image bit is almost redundant, I think 'Position' would be enough.
It might even be better to arrange the dialog with the

I'd change this to something like

Position Image
() relative to Paragraph
() relative to Column
() relative to Page

this doesn't sound quite right.

Taking a look at how Microsoft Word does this would definately be a very
good idea. You were definately right, there is definately something off
and inconsistant about the layout of the dialog but I'm not sure I've
done the best job of saying exactly what it is or how best to fix it.
