This has always been an FAQ. :)

I think almost every user encounters this at least once (well maybe not
old school Unix types who have already learnt what the Insert key is for
so long ago that they dont even remember learning it).
It is particularly difficult to figure out on your own what happened and I
think it is just something you have to learn.

I'm pretty sure I pissed off the developers no end until they eventually
implemented the cursor change to make this more obvious but I am very glad
they made the change (I also suggested disabing the feature entirely but
user assured us that some of them used it).
